Once you complete this Student Registration form you will be redirected to the Summer Camp Product Gallery where you can select your camp(s) and proceed to payment and checkout. 

YOUR REGISTRATION PROCESS IS NOT COMPLETE UNTIL YOU COMPLETE YOUR PAYMENT!

YPPC relies on Grants to provide theatre classes at the lowest price possible. To continue to receive grants, we must request participant demographics.

Does your child have any allergies, epi pen, or medications we should be aware of? 

We teach and practice safety protocols at YPPC. In the event your child becomes ill or is injured during class and you are not reachable, please let us know which treatments you approve. Please check all that apply.

Emergency Treatment Required

Does your child have any dietary restrictions we should be aware of?

Does your child have any behavioral, developmental, or emotional challenges that we should be aware of?

Is there anything else about your child that you think would be useful for us to know? 

Weather permitting, we will walk to Central Park for lunch. Do you give permission for teachers to take your child off campus?

We do our best to accommodate students with special needs, however we are not formally trained in this area. We ask that parents of children with significant social, behavioral, or developmental challenges be on the premises during class. Or even better, become a class assistant.

Confirmation & Signature

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
